MICROBIOLOGICAL PROFILE IN THERAPEUTIC KERATOPLASTY USING GAMMA IRRADIATED CORNEAL TISSUES

Abstract
Purpose: To evaluate the microbiological profile of severe keratitis managed with therapeutic keratoplasty using gamma-irradiated cornea tissues. Methods: A prospective case series of patients with severe corneal ulcers treated with therapeutic keratoplasty using gamma-irradiated cornea tissues at Ho Chi Minh City Eye Hospital from November 2018 to August 2023. Demographics, clinical exam findings, microbiological profile and treatment outcomes were collected and analyzed. Results: 33 eyes of 33 patients with folow-up time ranging from 3 to 24 months were evaluated. Fungi (66,7%) were the most common pathogen, followed by bacterial and HSV-1 (18,2% and 15,1% respectively). 90,5% fungal cases were confirmed by smear and pathology results. PCR detected 1 microsporidia case and 2 out of 5 cases originated from HSV-1. Clinical pathogonomic findings are helpful in bacterial (100%) and HSV-1 (60%) cases. 32 out of 33 eyes retained integrity at the end of the study. Conclusions: Determining the exact pathogens and performing therapeutic keratoplasty using gamma-irradiated cornea tissues at an approriate time are essential factors in predicting the outcome of severe corneal ulcers.
