RESULTS OF HYPERTENSION, DIABETES AND ISCHEMIC HEART DISEASE TREATMENTS IN THE ELDERLY AT THE FAMILY MEDICINE CLINIC - LE VAN THINH HOSPITAL

Abstract
Background: The disease spectrum in our country is changing with an increase in chronic non-communicable diseases, especially in the elderly, increasing the burden of disease on patients, families and society. Managing chronic non-communicable diseases at the primary care level through the family doctor clinic is a solution that both reduces the load on hospitals level and contributes to long-term health care and management for the elderly. Objective: Survey on management of common chronic non-communicable diseases for the elderly at the Family Doctor Clinic of Le Van Thinh Hospital in 2022. Subjects and research methods: This is a cross-sectional study. All elderly with at least one of the following diseases: Hypertension, Type 2 Diabetes, Ischemic Heart Disease who were examined at the Family Doctor Clinic of Le Van Thinh Hospital in 2022 were recruited. Results: The total number of elderly coming for examination in 2022 is 4713 people, of which 3506 people (74.4%) have at least one disease: hypertension, type 2 diabetes, ischemic heart disease. The rate of follow-up adherence is 85%. The majority of patients achieved their blood pressure and blood sugar goals and had no angina attacks during the year. The rate of patients with complications is 3.8%. Conclusion: The survey shows that the prevalence of hypertension, type 2 diabetes, and ischemic heart disease are predominant. Most patients achieve treatment goals; the complication rate is low, although the follow-up examination is still not regular.
