30-DAY PROGNOSTIC VALUE OF ALBUMIN-BILIRUBIN INDEX IN CIRRHOSIS PATIENTS WITH GASTROINTESTINAL BLEEDING DUE TO PORTAL HYPERTENSION

Abstract

Objective: Survey of the 30 day prognostic value of the ALBI index with MELD and Child-Pugh scales. Materials and Methods: 171 patients with cirrhosis with gastrointestinal bleeding due to increased venous pressure are admitted for treatment at the Center for Gastroenterology and Hepatology – Bach Mai Hospital from 11/2021 to the end 06/2022. Evaluate ALBI, MELD and Child-Pugh within the 24 hours and monitor recurrent hemorrhage and death for 30 days following hospitalization. Results: The area below the curve (AUC) for the 30-day prognosis of ALBI, MELD and Child-Pugh indexes was 0,769; 0,696 and 0,718 respectively. The cut-off values of ALBI, MELD and Child-Pugh in the 30-day re-hemorrhage prognosis were -1,495, respectively; 14,5 and 7,5. For the prognosis of death the AUC of ALBI, MELD and Child-Pugh was 0.925; 0,882 and 0,915, respectively. The cut-off values of ALBI, MELD and Child-Pugh in the 30-day prognosis of death were -0,97, respectively; 18,5 and 10,5. Conclusion: The ALBI index is useful in evaluating short-term outcomes of cirrhosis patients with gastrointestinal bleeding due to portal hypertension, with better performance than MELD and Child-Pugh scales.
